An opportunity has ariseno  for a full-time social care practitioner within the North and West Community Team 2 on a fixed term basis as a secondment. 

We are a team of Social workers, Social care practitioners, Senior Practitioners and a Team Manager based in Southmead House, who aims to promote the choice and control for citizens and their carers, enabling them to live as independently as possible and free from harm or abuse.

Your role would be to work under the statutory requirements of the Care act 2014 to undertake support conversations, strength based Care Act assessments, Carers Assessments and reviews with citizens in need of support and help them to access appropriate support in line with the department’s policies and procedures under the supervision of a Manager, Senior Practitioner or Social Worker.

You will be required to undertake support planning with service users who has eligible care and support needs to access the support they need and to meet agreed outcomes that promote choice and control and help them remain independent.

As part of your role, you will be undertaking Mental Capacity Assessments and Best Interests within the statutory requirements of the Mental Capacity Act 2005 with the support of qualified Social Worker where required.


You will be expected to contribute to investigations of abuse, as directed by a manager/social worker and to make appropriate Safeguarding alerts in line with Bristol’s Safeguarding Adults policy. You will be participating in duty rotas responding to unplanned/emergency work that does not require a qualified Social Worker on a regular basis.

You are required to travel mainly within the City Council boundaries to attend meetings, visit service users and on occasion to transport service users. Any employee who, as a result of their disability is unable to drive will be provided with transport in the course of their duties at no personal cost.

Please note that this role is subject to an Enhanced DBS Check.

Qualifications:

Educated to NVQ level 3 or significant relevant experience and evidence of
commitment to own personal and professional development.
Experience of working without direct supervision.
Worked (voluntary or paid) with adults and carers in community care, health or a
closely related field.
